State revokes, suspends licenses, certifications, registrations of health care providers

The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

King County

In January 2007 the Physical Therapy Board revoked the license of physical therapist James G. Aiton (PT00005217) for 10 years. He was convicted of theft.

In January 2007 the Unlicensed Practice Program entered into an Agreed Order to Cease and Desist with Dae Kim. She agrees to stop the unlicensed practice as a dental hygienist. Kim performed ultrasonic cleaning and scaling at a dental facility.

In December 2006 the Nursing Assistant Program revoked the credential of nursing assistant Kimberly M. Jones (NC10080546) for five years. She hit a patient on the head with her hand.

In December 2006 the Nursing Assistant Program indefinitely suspended the credential of nursing assistant Gordon A. Roe (NC10088949). He failed to properly assist patients with hygiene and toiletry needs and caused a patient to bruise.

In January 2007 the Nursing Commission indefinitely suspended the license of registered nurse Patrick M. Shaffer (RN00148173). He was convicted of grave stealing and failed a drug screen.

In December 2006 the Nursing Assistant Program revoked the credential of nursing assistant Lonnie B. Stephenson (NC10028295) for 10 years. He crossed professional boundaries with patients by massage and touching them inappropriately.

Pierce County

In January 2007 the Nursing Assistant Program revoked the credential of nursing assistant Dolores L. Chandler (NC10062939) for 10 years. She was placed on the OBRA Nurse Assistant Registry by the Department of Social and Health Services and she may not care for vulnerable adults. Chandler stole money from a patient.

In January 2007 the Nursing Assistant Program revoked the credential of nursing assistant Charles K. Kimball (NA00179462) for two years. He slapped a patient and hit another. The Department of Social and Health Services issued a Notice of Finding against Kimball and he is restricted from working in a nursing home or treating vulnerable adults.

In December 2006 the Department of Health suspended the license of chiropractor Bruce Michael Linehan (CH00002852). He failed to pay a student loan.

In January 2007 the Registered Counselor Program entered an Agreed Order with registered counselor David B. Ricker (RC00018787). He agrees to surrender his registration and not practice as a registered counselor. He failed to reveal a criminal conviction when he applied for registration.

Skagit County

In January 2007 the Nursing Commission indefinitely suspended the license of registered nurse Carol Diane Mackey (RN00103283). She arrived for work under the influence of alcohol.

Snohomish County

In January 2007 the Nursing Commission suspended the license of Elzbieta E. Archibald, a licensed practical nurse (LP00040513). She failed to comply with the terms and conditions of a prior order.

In January 2007 the Nursing Assistant Program revoked the credential of nursing assistant Paul Barton (NC10093208) for 20 years. He touched patients inappropriately and made sexual comments to patients. His credential was immediately suspended in earlier this month.

Spokane County

In January 2007 the Nursing Assistant Program revoked the registration of nursing assistant Duane N. Thomas (NA00135240) for 10 years. He was convicted of distributing cocaine and intending to distribute cocaine.

Thurston County

In December 2006 the Nursing Commission indefinitely suspended the license of registered nurse Cheryl H. Westergaard (RN00040824). She failed to comply with terms and conditions against her license.

Whatcom County

In January 2007 the Unlicensed Practice Program issued a Cease and Desist Order against Jiachang Pu. Pu must stop the unlicensed practice of massage. Pu provided chair massage on customers at the Bellis Fair Mall in Bellingham. Jaichang Pu is also known as Jai Chang Pu.

In January 2007 the Registered Counselor and Mental Health Counselor Programs indefinitely suspended the credentials of Christine Steckel, a registered counselor and mental health counselor (RC00030568, LH00006772). She failed to comply with the terms and conditions of a prior order.

Yakima County

In January 2007 the Nursing Commission suspended the license of registered nurse Sarah E. Glasscock (RN00160178). She failed to comply with the terms and conditions of a prior order and she must comply before she can petition for reinstatement of her license.

Out of State

Colorado: In December 2006 the Nursing Commission indefinitely suspended the license of Dena G. Garcia, a registered nurse licensed in Washington (RN00151631). Her Arizona nursing license was revoked in 2006. She was immediately suspended by the Washington commission in October 2006.

Colorado: In January 2007 the Unlicensed Practice Program issued a Cease and Desist Order against Kaila Swallow. She practiced as a nursing assistant in Washington after her nursing assistant registration had expired.

Florida: In January 2007 the Medical Commission entered into an Agreed Order with Britt M. Borden. He agrees not to renew his license to practice as a physician in Washington (MD00035869).

Florida: In January 2007 the Medical Commission entered into an Agreed Order with Jeffery A. Clark. He agrees not to renew his license to practice as a physician in Washington (MD00038159).

Tennessee: In January 2007 the Nursing Commission indefinitely suspended the license of Charmaine M. Garner, a registered nurse licensed in Washington (RN00139503). She had disciplinary action taken against her Arizona nursing license and she is prohibiting from practicing in Arizona for five years.
