"Chronic Disease Management: Evidence of Predictable Savings," Health Management Associates: The study finds that targeted and well-designed care management programs can improve health outcomes for people with chronic diseases and save money on a predictable basis.


The report also identifies key elements of care management interventions that increase the chances that such programs will succeed (Meyer/Smith, "Chronic Disease Management: Evidence of Predictable Savings," November 2008).
