The Washington department Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Grays Harbor County

In April 2009 the Nursing Assistant Program denied the application of Dallas Dennis Devoron Greer (NA60076034). He was convicted of two counts of custodial sexual misconduct.

King County

In April 2009 the Board of Pharmacy charged AMS Home Care USA, Inc, (CF00058234) and its president and CEO Rani Gill and Harj Gill with unprofessional conduct. They did business as 65 PLUS in Bellevue and closed the pharmacy in September 2007. They allegedly failed to notify the department of their intent to cease operations, failed to submit required documentation, and left drugs, order forms, and prescription pads on the premises until a formal eviction occurred a year later.

In May 2009 the Board of Nursing Home Administrators terminated the order of probation against Moe S. Chaudry (NH00001492).

In May 2009 the Dental Commission released dentist Kam Derakshani (DE00007712) from terms and conditions set against his license.

In May 2009 the Physical Therapy Program granted the application of physical therapist assistant David J. Helde (P160055702) and placed his license on probation for two years. The Oregon Physical Therapist Licensing Board also placed his license to practice on probation for two years. He must comply with terms and conditions set against his license.

In April 2009 the Department of Health charged physician Geoffrey A. MacPherson (MD00011292) with unprofessional conduct. He allegedly engaged in sexual misconduct with the mothers of two patients.

In April 2009 the Massage Therapy Program granted the application of LaTanya N. Moore (MA60019056) and placed her license on probation for one year. She was convicted of cocaine possession and delivery of cocaine. She must comply with terms and conditions set against her license.

In April 2009 the Registered Counselor Program denied the application of Tyler G. Petrosky (RC60036355). He did not demonstrate he is qualified to practice.

In April 2009 the Health Care Assistant Program granted the application of Michael John Prado (HC00146796) and placed his license on probation for one year. In July 2008 his request to be licensed as a health care assistant and x-ray technician was denied based on a felony conviction. He must comply with terms and conditions set against his license.

In May 2009 the Nursing Assistant Program granted the registration and certification applications of certified nursing assistant Peiti Tung (NA60059278, NC60069069) and placed them on probation for two years. Tung was convicted of mail fraud. She must comply with terms and conditions set against her registration and certification. She is also known as Marjorie M. Tung.

Pierce County

In May 2009 the Dental Commission entered into an agreed order with dentist Debra Hopkins (DE00006126). The care she provided fell below the standard while placing restorations on a patient’s teeth. She must comply with terms and conditions set against her license.

In April 2009 the Nursing Assistant Program charged certified nursing assistant Lillian M. Kroll (NC10069892) with unprofessional conduct. She allegedly borrowed money from a patient.

In April 2009 the Board of Pharmacy charged pharmacy technician Jennifer L. Lake (VA00062699) with unprofessional conduct. She allegedly filled and refilled a prescription for her own use without authorization, and admitted diverting a controlled substance for her personal use.

In April 2009 the Nursing Commission released licensed practical nurse Maria E. Lengele (LP00055419) from terms and conditions set against her license.

Skagit County

In April 2009 the Nursing Assistant Program denied the application of Jaime Malagayo

Antenor (NA60069827). He was convicted of assault – domestic violence, and failed to disclose the conviction on his application.

Snohomish County

In April 2009 the Nursing Assistant Program denied the application of Michelle Marie Bradshaw (NA60063394). Her license to practice as a licensed practical nurse was indefinitely suspended.

In April 2009 the Massage Practitioner Program charged Brian J. Lucas (MA00011792) with unprofessional conduct. During a massage treatment, he allegedly exposed a patient’s breasts, slid his hand under her underpants, and felt her genitals.

In April 2009 the Board of Physical Therapy charged Suzann Oakes (PT00002013) with unprofessional conduct. She allegedly had numerous billing errors in her records, forged other therapists’ notes, and had someone forge a Medicare form for her.

Spokane County

In April 2009 the Nursing Assistant Program charged Heather Y. Colwell (NA00144215) with unprofessional conduct. She failed to respond to department inquiries regarding an allegation about stealing medication.

In May 2009 the Registered Counselor Program terminated the order of probation against Marc D. Costello (RC00055369).

In May 2009 the Registered Counselor Program terminated the order of probation against Susan L. Duval (RC00055985).

In May 2009 the Dental Commission released dentist Frank F. Vedelago (DE00002897) from terms and conditions set against his license.

In May 2009 the Registered Counselor Program granted the application of Anthony Lee Williams (RC60068273) and placed his registration on probation for three years. He was convicted of burglary, possession of cocaine, robbery on more than one occasion, and conspiracy to deliver cocaine on multiple occasions. He must comply with terms and conditions set against his registration.

Thurston County

In May 2009 the Health Care Assistant Program granted the application of Tracy Lee Anderson (HC60036656) and placed her license on probation for two years. She was convicted of

malicious mischief. She must comply with terms and conditions set against her license.

In May 2009 the Chemical Dependency Professional Program granted the application of Ruth Mildred Wolbert-Neff (CP60080400) and placed her license on probation for two years. She was convicted of unlawful possession of a controlled substance – methamphetamine. She must comply with terms and conditions set against her license.

Whatcom County

In April 2009 the Nursing Commission placed the license of registered nurse Michelle Frank (RN00120509) on probation for two years. She routinely pre-poured and pre-charted administration of medications. She must comply with terms and conditions set against her license.

In May 2009 the Registered Counselor Program granted the application of Faisal A. Roberts (RC60056468) and placed his registration on probation for two years. He was convicted of theft and must comply with terms and conditions set against his registration.

In May 2009 the Registered Counselor Program released Tamara S. Zaferatos (RC00045084) from terms and conditions set against her registration.

Yakima County

In April 2009 the Nursing Commission charged registered nurse Kristal L. Donaldson (RN00148938) with unprofessional conduct. She allegedly failed to comply with terms and conditions set against her license.
