Washington Revokes, Suspends Licenses Of Health Care Providers

The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Systems Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency's Web site. Click on "Provider Credential Search" on the left hand side of the Department of Health home page doh.wa.gov . The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Chelan County

In October 2008 the Registered Counselor Program suspended the credential of E. Stanley Foster (RC00054054). Foster's actions could have made clients uncomfortable or created misunderstanding.

Clallam County

In October 2008 the Nursing Commission withdrew the statement of charges against registered nurse Marsi L. Smith (RN00131782).

Clark County

In October 2008 the Pharmacy Board accepted the license surrender of pharmacy technician Arleea Dobson (VA00049937). Dobson diverted controlled substances from her place of employment for her own use.

In October 2008 the Pharmacy Board permanently revoked the credential of pharmacy assistant Judy Melgoza (VB00070306). Melgoza diverted drugs for her own use and stole more than $4,000 worth of drugs and merchandise from her place of employment.

Franklin County

In October 2008 the Nursing Commission indefinitely suspended the license of registered nurse Darryl K. Jones (RN00121009). Jones failed to comply with previous terms set against his license.

Island County

In October 2008 the Dental Commission indefinitely suspended the license of dentist Michael W. Ostrom (DE00003981). Ostrom drank alcohol at work and then performed dentistry on a patient.

Jefferson County

In October 2008 the Veterinary Board of Governors placed the license of Virginia M. Johnson (VT00002875) on stayed suspension and required compliance with terms and conditions. Johnson participated in importing two puppies without taking required steps to protect the public from rabies.

In October 2008 the Veterinary Board of Governors placed the credential of Joyce M. Murphy (VT00001824) on stayed suspension and required compliance with terms and conditions. Murphy imported two puppies without taking required steps to protect the public from rabies.

King County

In October 2008 the Registered Counselor Program suspended the credential of Audrey Buckton (RC00047846). Buckton began a sexual and/or romantic relationship with a client.

In October 2008 the Health Care Assistant Program indefinitely suspended the credential of Victoria J. Graham (HC00141626). Graham failed to comply with previous terms set against her credential.

In October 2008 the Pharmacy Board revoked the credential of pharmacy technician Kimberly A. Suedel (PTEC.VA.00050302). Suedel diverted drugs from her place of employment.

In October 2008 the Nursing Assistant Program indefinitely suspended the credential of certified nursing assistant Isayas M. Worku (NC10082470). Worku was convicted of second degree assault.

Kitsap County

In October 2008 the Nursing Commission indefinitely suspended the credential of registered nurse Constance C. Staggs (RN00107198). Staggs failed to cooperate with an investigation.

Pierce County

In October 2008 the Dental Commission accepted the license surrender of dentist Robert L. Kiehl (DE00003766). Kiehl cannot practice dentistry in Washington.

In October 2008 the Massage Program revoked the credential of Edward Kim (MA00013024). Kim sexually touched a patient during a massage and pleaded guilty to a sex offense relating to the incident.

In October 2008 the Respiratory Care Program suspended the credential of Robert G. Marengo (LR00002364). Marengo was convicted of two counts of fourth degree assault. He was also convicted of unlawful possession of a controlled substance – cocaine.

In October 2008 the Nursing Commission indefinitely suspended the license of licensed practical nurse Robin Tate (LP00039802). Tate failed to comply with terms set against her license.

Snohomish County

In October 2008 the Nursing Commission withdrew the statement of charges against licensed practical nurse Nusrat Bhatti (LP00051614).

In October 2008 the Registered Counselor Program indefinitely suspended the credential of Lee Coffey (RC00051932).

In October 2008 the Registered Counselor and Chemical Dependency Professional Programs withdrew the statement of charges against Brian Hall (RC00044890, CP00005232).

In October 2008 the Pharmacy Board revoked the credential of pharmacy technician Tyler H. Ledford (PTEC.VA.00042955). Ledford diverted drugs from his place of employment for his own use. He did this by forging about 94 prescriptions.

Spokane County

In October 2008 the Health Care Assistant Program indefinitely suspended the credential of Stephanie A. Holloway (HCA.HC.60022674). Holloway failed to comply with previous terms set against her credential.

In October 2008 the Nursing Assistant Program suspended the credential of certified nursing assistant Isaac Ray Mattews (NAC.NC10044039). Matthews was convicted of two counts of third degree assault and failed to respond to department questions about the convictions.

In October 2008 the Nursing Assistant Program suspended the credential of Dianne J. Melton (NA00199207). Melton did not use the standard protocol for dealing with a difficult and combative patient.

Thurston County

In October 2008 the Dental Commission withdrew the statement of charges against dentist Edward A. Fletcher (DE00004710).

Whatcom County

In October 2008 the Registered Counselor Program indefinitely suspended the credential of Peggy L. Shea (RC00057571).

Yakima County

In October 2008 the Secretary of Health withdrew the statement of charges against registered counselor Leanna Blue (RC.RC00047139).

Out of State

Illinois: In October 2008 the Nursing Commission indefinitely suspended the license of registered nurse Janice L. Kelly (Rowark) (RN00162302). Kelly documented that she had changed a patient's wound dressing when she had not.

Oregon: In October 2008 the Nursing Commission indefinitely suspended the license of licensed practical nurse Sharon R. Simonds (LP00043381). Simonds failed to comply with a previous order.
