Washington Revokes, Suspends Licenses Of Health Care Providers

The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Professions Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency's Web site. Click on "Provider Credential Search" on the left hand side of the Department of Health home page. The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In June 2008 the Registered Counselor Program suspended the credential of Deana M. Eley (RN00162762). Eley failed to comply with a previous order.

Island County

In June 2008 the Physical Therapy Board indefinitely suspended the credential of Perry Gordon (PT00010404). Gordon tested positive for cocaine during a pre-employment drug screening.

King County

In June 2008 the Nursing Commission indefinitely suspended the license of registered nurse George L. Armour (RN00053309). Armour failed to comply with a previous order.

In June 2008 the Nursing Assistant Program suspended the credential of Tracy Backman (NC10073904). Backman failed to respond to department inquiries regarding a complaint that she had abused a resident.

In June 2008 the Nursing Commission suspended the credential of registered nurse Joanne G. Guerrero (RN00142855). Guerrero made entries on her time card resulting in over-payments totaling a minimum of $84,753.90.

In June 2008 the Nursing Commission indefinitely suspended the license of registered nurse Donna S. Hamshar (RN00118448). Hamshar's credential to practice as a registered nurse was permanently revoked by the Alaska Board of Nursing.

In May 2008 the Nursing Assistant Program revoked the credential of Tracy A. Johnson (NC10071310). Johnson was convicted of first and second degree assault.

In June 2008 the Nursing Assistant Program indefinitely suspended the credential of certified nursing assistant Lynn Knutz (NC10101466). Knutz failed to comply with previous terms set against her credential.

In June 2008 the Medical Commission dismissed the amended statement of charges against physician William H. Levy (MD00021325).

In June 2008 the Board of Physical Therapy suspended the license of Gary J. Maguire (PT00005702). Maguire had a sexual relationship with two patients.

In June 2008 the Registered Counselor Program suspended the credential of Natalie N. McKinley (RC00052638). McKinley became romantically involved with a patient and married the patient. She is also known as Natalie N. Gomez.

In June 2008 the Registered Counselor Program suspended the credential of Junne Y. Park (RC00050216). Park failed to comply with a previous order.

In June 2008 the Registered Counselor Program suspended the credential of Gerard C. Prestwich (RC00056469). Prestwich failed to comply with a previous order.

In June 2008 the Chemical Dependency Professional and the Licensed Mental Health Counselor Programs indefinitely suspended the mental health counselor license of Carol Van Buren (CP00000124, LH00010800). She retains her chemical dependency professional credential. Van Buren submitted a falsified postgraduate experience verification form when she applied for the mental health counselor license.

Mason County

In June 2008 the Medical Commission indefinitely suspended the license of Donald R. Miller (MD00026759). Miller failed to respond to department inquiries regarding patient complaints.

Pierce County

In June 2008 the Nursing Commission withdrew charges against Joyce L. Butler (RN00079668).

In June 2008 the Board of Physical Therapy withdrew the statement of charges against physical therapist Kurt Moss (PT00004067).

In June 2008 the Nursing Commission revoked the license of registered nurse Dana Lynn Ward (RN00116287). Ward was convicted of possession of a controlled substance and failed to respond to investigator inquiries about the conviction.

Skagit County

In June 2008 the Nursing Assistant Program revoked the credential of Bryan E. Cooper (NC10042521). Cooper failed to transfer a patient using a two-person transfer as required in the patient's care plan and this resulted in the patient breaking two bones.

In June 2008 the Nursing Assistant Program suspended the credential of Sheril M. Salgado (NC10064202). Salgado was convicted of third degree theft and making a false or misleading statement to a public official.

Spokane County

In June 2008 the Registered Counselor Program suspended the credential of Amber M. Barnes (RC00054889). Barnes failed to comply with a previous order.

In June 2008 the Secretary of Health withdrew charges against Krystel C. Nealand (NC10083790).

Stevens County

In June 2008 the Nursing Commission vacated the summary suspension order and withdrew the statement of charges against registered nurse and advanced registered nurse practitioner David A. Peck (RN00077493, AP30003820).

Thurston County

In May 2008 the Secretary of Health withdrew the statement of charges against nursing assistant Darcy Lockard (NC10041100).

Whatcom County

In June 2008 the Nursing Commission indefinitely suspended the credential of Jeanette Anne Hunt (RN00061408). Hunt failed to comply with previous terms set against her credential.

In June 2008 the Nursing Assistant Program indefinitely suspended the credential of Dennis S. Pratt (NA00167185). Pratt failed to comply with previous terms set against his credential.

In June 2008 the X-Ray Technician Program suspended the credential of Jennifer L. Wolford (XT00005315). Wolford's licensed practical nursing license was suspended following her conviction for attempted forgery.

Yakima County

In June 2008 the Nursing Assistant Program suspended the credential of Blanca M. Mendoza (NA00195661, ST00003787) and her application for a surgical technologist credential was denied. Mendoza was convicted of first degree theft and attempted forgery.

Out of State

Arizona: In June 2008 the Nursing Commission indefinitely suspended the license of registered nurse Shawn E. Nyman (RN00127983). Nyman's credential to practice as a registered nurse was revoked by the Arizona State Board of Nursing.

Illinois: In June 2008 the Nursing Commission indefinitely suspended the license of licensed practical nurse Timothy E. Charters (LP00049088). Charters' license to practice as a licensed practical nurse in Oregon was revoked by the Oregon Board of Nursing for verbal abuse of patients and patients' family members, threatening and intimidating co-workers, and failure to comply with the board.

Louisiana: In June 2008 the Nursing Commission indefinitely suspended the license of registered nurse Jody R. Hart (RN00175712). Hart's credential to practice as a registered nurse was suspended by the Louisiana Board of Nursing.

Montana: In June 2008 the Nursing Commission indefinitely suspended the credential of registered nurse Jacqueline K. Sonmez (RN00134263). Sonmez' license to practice as a registered nurse in Montana is suspended.

Oklahoma: In June 2008 the Nursing Commission withdrew the statement of charges and vacated the summary suspension against Walter T. DeLong (RN00150454).

Oregon: In June 2008 the Nursing Commission indefinitely suspended the license of registered nurse Gretchen M. Lehrer (RN00144708). Lehrer surrendered her credential to practice as a registered nurse to the Oregon State Board of Nursing.
