Washington Revokes, Suspends Licenses Of Health Care Providers

The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Professions Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency’s Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In June 2008 the Nursing Commission suspended the license of registered nurse Jennifer A. Green (RN00111529). Green was convicted of first degree criminal mistreatment.

Island County

In May 2008 the Office of Emergency Medical Services and Trauma System revoked the certification of Tracy R. Adams (01167215). Adams was convicted of seven counts of first degree computer trespass.

King County

In May 2008 the Nursing Assistant Program suspended the credential of Jessie R. Adaya (NA00182965). Adaya applied for his nursing assistant credential using a fraudulent social security number. He also used the number while employed at an adult family home.

In May 2008 the Nursing Assistant Program revoked the credential of Tyler R. Bonus (NA00190946). Bonus quit his job during his shift, abandoning six vulnerable adult patients. He also had health care information regarding patients on his personal laptop and wrote a false doctor’s order for a patient.

In May 2008 the Registered Counselor and Chemical Dependency Professional Programs suspended the credentials of Natalie B. Banks (CP00000101, RC00019437). Banks failed to comply with a previous order.

In June 2008 the Health Care Assistant Program indefinitely suspended the credentials of Karolyn K. Lee (HC00141120, HC00150064). Lee failed to comply with terms set against her credentials.

In May 2008 the Nursing Assistant Program suspended the credential of Maria E. Leonardo (NA00187614). Leonardo applied for her nursing assistant credential using a fraudulent social security number. She used the number while employed at an adult family home.

In June 2008 the Nursing Assistant Program indefinitely suspended the credential of Nelson R. Lopecillo (NA00190366). Lopecillo applied for his nursing assistant credential using a fraudulent social security number. He also used the number while employed at an adult family home.

In May 2008 the Nursing Assistant Program suspended the credential of certified nursing assistant Susan C. Seaman (NC10056111). Seaman failed to comply with terms set against her credential and was convicted of third degree theft.

Kitsap County

In May 2009 the Nursing Assistant Program suspended the credential of certified nursing assistant Carl T. Carter (NC10101727). Carter failed to comply with terms set against his credential.

Mason County

In June 2008 the Nursing Commission revoked the license of registered nurse Diana Lancaster (RN00071691). Lancaster was convicted of acquiring a controlled substance by deception.

Pacific County

In June 2008 the Nursing Assistant Program revoked the credential of certified nursing assistant Jenny Hanna (NC10056995). Hanna was convicted of first degree criminal trespass and third degree domestic violence theft.

Pierce County

In June 2008 the Nursing Commission suspended the license of registered nurse Sharleen A. Conner (RN00106814). Conner stole medication from a patient and failed to respond to department inquiries regarding the incident.

In June 2008 the Nursing Assistant Program indefinitely suspended the credential of Kashonder C. Cotton (NC10102213). Cotton did not follow a patient’s care plan when transferring the patient.

In May 2008 the Nursing Commission suspended the license of licensed practical nurse Alice N. Currie (LP00050270). Currie failed to comply with terms set against her license.

In May 2008 the Chemical Dependency Professional Program suspended the credential of Anthony M. Hay (CP00004687). Hay reported two people were clients of the agency where he worked and had complied with court-ordered conditions. Some of the reports were made on agency letterhead after he left the agency. The agency had no treatment records about those clients.

In June 2008 the Nursing Assistant Program suspended the credential of Maria L. Magaway (NA00180018). Magaway practiced beyond her scope when she administered insulin injections to a patient.

In June 2008 the Nursing Commission suspended the license of licensed practical nurse Teresa J. Miles (LP00045536). Miles failed to comply with a previous terms set against her license.

In May 2008 the Pharmacy Board revoked the credential of pharmacy technician Lara E. Stewart (VA00016710). Stewart stole drugs from her place of employment. She was arrested for it in September 2007 and admitted to stealing drugs for personal use since the summer of 2005.

Snohomish County

In May 2008 the Pharmacy Board revoked the credential of pharmacy technician Carmen M. Olson (CA00053926). Olson stole 20 bottles of hydrocodone/acetaminophen from her place of employment.

In May 2008 the Nursing Commission indefinitely suspended the licensed of licensed practical nurse Teresa L. Page (LP00035469). Page failed to comply with terms set against her license.

In May 2008 the Secretary of Health withdrew charges against nursing assistant Natasha N. Stewart (NC10094915).

Spokane County

In June 2008 the Nursing Assistant Program revoked the credential of certified nursing assistant Joshua C. Hooper (NC10065365). Hooper stole prescription drugs and money from a patient’s wife.

Thurston County

In May 2008 the Secretary of Health withdrew the statement of charges against nursing assistant Darcy Lockard (NC10041100).

Yakima County

In June 2008 the Emergency Medical Services and Trauma System Program revoked the credential of emergency medical technician Robert J. Murdock (ES01170655). Murdock was convicted of third degree assault with sexual motivation.

In June 2008 the Surgical Technologist Program suspended the credential of Basilisa Pacheco (ST00000927). Pacheco forged a prescription for her own use. She also left work without notifying a supervisor prior to a surgery putting a patient at risk of harm.

Out of State

Colorado: In June 2008 the Nursing Commission summarily suspended the license of registered nurse Rachel Ann Beck (RN00174654). Beck's license to practice as a registered nurse was suspended by the Colorado State Board of Nursing.

Idaho: In June 2008 the Pharmacy Board summarily suspended the credential of pharmacist Jason Jungert (PH00039062). Jungert’s pharmacy license was indefinitely suspended by the Idaho Board of Pharmacy for diverting controlled substances from his employer.

North Carolina: In May 2008 the Medical Commission summarily suspended the license of physician Eric F. Strother (MD00036141). Strother surrendered his license to practice medicine to the North Carolina Medical Board.

Oklahoma: In June 2008 the Nursing Commission summarily suspended the license of registered nurse Walter T. Delong (RN00150454). Delong’s license to practice as a registered nurse was revoked by the Oklahoma Board of Nursing for failing to comply with an order.

Oregon: In May 2008 the Nursing Commission revoked the license of registered nurse Susan D. Fast (RN00125856). Fast stole over $28,900 from her employer and was convicted of first degree theft and aggravated theft.

Oregon: In June 2008 the Pharmacy Board summarily suspended the credential of pharmacy assistant Michael J. Sheppy (VB00071812). Sheppy surrendered his credential to practice as a pharmacy assistant to the Oregon State Board of Pharmacy.
