Washington Suspends Licenses Of Health Care Providers

The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Systems Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Benton County

In August of 2009 the Medical Quality Assurance Commission withdrew a statement of charges against physician Thomas R. Burgdorff (MD00018412).

Grant County

In August of 2009 the Nursing Care Quality Assurance Commission suspended the credential of registered nurse Connie J. Raphael (RN.RN.00114309) for at least five years. Raphael was convicted in Benton and Grant County superior and district courts of two Class C felonies and six gross misdemeanors including driving under the influence, reckless driving, assault, and violation of a protection order.

King County

In August of 2009 the Department of Health revoked the chemical dependency professional and licensed mental health counselor credentials of Geoffrey J.L. Chipps (CDP.CP.00002870, MHC.LH.00007078). Chipps had a sexual relationship with one client and engaged in inappropriate behavior with others.

Kitsap County

In September of 2009 the Registered Nursing Assistant Program suspended the credential of Jordan T. Henzel (NAR.NA.60027093), who was convicted in Kitsap County Superior Court of delivery of marijuana, a Class C felony.

Mason County

In September of 2009 the Dental Quality Assurance Commission granted a dental assistant credential to Melanie D. Enger (DEAS.D1.60050041) and placed her on probation for at least 18 months. Enger was convicted of driving under the influence.

Pierce County

In July of 2009 the Nursing Assistant Program suspended for at least three years the credential of Damnal Kang (NAC.NC.10072725) because he was convicted in Pierce County Superior Court of third-degree assault, a Class C felony.

In August of 2009 the Nursing Assistant Program suspended the credential of Katrina G. Allen (NAC.NC.10087230) for failure to comply with terms of an earlier order.

In August of 2009 the Medical Quality Assurance Commission withdrew a statement of charges against physician Robert W. Kunkle (MD00028126).

Skagit County

In August of 2009 the Nursing Care Quality Assurance Commission suspended the credential of registered nurse Jolynne P. Brooks (RN.RN.00059030) for failure to comply with an earlier order.

Snohomish County

In August of 2009 the Department of Health suspended the credential of massage practitioner Robin Haight (MASS.MA00021195) for failure to comply with a child support order. The suspension will continue until Haight complies with the support order.

Spokane County

In August of 2009 the Nursing Care Quality Assurance Commission dismissed an unprofessional conduct charge against registered nurse Linda J. Cordoror (LP00041310).

In August of 2009 the Health Care Assistant Program suspended the credential of Andrew L. Mourning (HCA.HC.00154439) because he was convicted in Spokane County Superior Court of one count of second-degree assault, a Class B felony, and in Tukwila Municipal Court of two counts of fourth-degree assault-domestic violence, gross misdemeanors.

In August of 2009 the Nursing Assistant Program suspended for at least three years the credential of Amanda M. Boisjolie (NAR.NA.00189023) because she was convicted in Spokane County Superior Court of one count of first-degree theft, a Class B felony, and two counts of third-degree assault, Class C felonies.

Whatcom County

In August of 2009 the Nursing Assistant Program suspended for at least 18 months the credential of Tracy L. Lorenz (NAR.NA.00095142) because she was convicted in Bellingham Municipal Court of third-degree theft, a gross misdemeanor.

In September of 2009 the Nursing Care Quality Assurance Commission suspended the credential of licensed practical nurse Leslie J. Gesner (LPN.LP.00050939), whose midwife credential was suspended in 2008.

Yakima County

In August of 2009 the Registered Counselor Program entered into an agreed order with Sally S. DeLeon (RC.RC.00040372) in which her credential is suspended for at least seven years. DeLeon claimed two children of a client's as dependents on her federal income tax return.

In July of 2009 osteopathic physician and surgeon Gary A. Beehler (DO.OP.00001310) agreed to surrender his credential to practice in Washington.

Out of State

Idaho: In August of 2009 the Nursing Assistant Program suspended the credential of Andrew T. Davie (NAC.NC.10101755) for failure to comply with an earlier order.

Maine: In September of 2009 the Medical Quality Assurance Commission summarily suspended the credential of physician Michael W. Blab (MD00032607) after the New York Department of Health's State Board for Professional Misconduct prohibited him from practicing in New York until he underwent a clinical competency assessment.

Maryland: In July of 2009 the Board of Pharmacy summarily suspended the credential of pharmacist Jeffrey P. Yannello (PHRM.PH.00018138) after the Pennsylvania State Board of Pharmacy suspended his credential to practice in that state.

Michigan: In July of 2009 the Nursing Care Quality Assurance Commission summarily suspended the credential of registered nurse Patricia J. Barrios (RN.RN.00139289) after she surrendered her credential to practice in Oregon and was prohibited from practicing there.

Minnesota: In July of 2009 the Nursing Care Quality Assurance Commission suspended the credential of registered nurse Matthew D. Lindemann (RN.RN.60014723) after he voluntarily surrendered his credential to practice in Wisconsin.

Oregon: In August of 2009 the Nursing Care Quality Assurance Commission summarily suspended the credential of registered nurse Shannon R. Stromberg (RN.RN.00164684) after the California Board of Registered Nursing revoked her credential to practice in that state.
