The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department's Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Chelan County
In December 2008 the Health Care Assistant Program charged Cynthia L. Reed (HC00144729) with unprofessional conduct. She allegedly felt a patient's veins with ungloved hands and failed to respond to department inquiries regarding the allegation.
Cowlitz County
In December 2008 the Registered Counselor Program granted the application of Angela M. Walborn (RC60036502) and placed her registration on probation for two years. She was convicted of possession of a controlled substance. She must comply with terms and conditions set against her registration.
King County
In December 2008 the Nursing Commission charged licensed practical nurse Darlene Bausch (LP00044484) with unprofessional conduct. She allegedly failed to ensure a patient's care plan was implemented which resulted in patient injury.
In December 2008 the Medical Commission reinstated the license of physician John J. Fina (MD00033991). Dr. Fina does not have restrictions against his license.
In December 2008 the Chemical Dependency Professional Program granted the application of Terrance Orlando Jones (CP60044697) and placed his license on probation for five years. His credential to practice as a registered counselor was placed on probation in 2007. He must comply with terms and conditions set against his license.
In December 2008 the Nursing Commission charged licensed practical nurse James R. Jovick (LP00040424) with unprofessional conduct. He allegedly failed to comply with terms and conditions set against his license.
In December 2008 the Nursing Assistant Program placed the certification of certified nursing assistant Marnette Keister (NC10069679) on probation for five years. She would have another caregiver care for her patients but created a timesheet showing she had cared for the patients. She must comply with terms and conditions set against her certification.
In December 2008 the Registered Counselor Program granted the application of Nicole T. Webb (RC60033077) and placed her registration on probation for two years. She was convicted of theft. She must comply with terms and conditions set against her license.
Kitsap County
In December 2008 the Massage Therapy Program granted the reactivation application of Mara C. Lindbergh (MA00006369) and placed terms and conditions on her license. She practiced massage therapy while her license was expired.
Okanogan County
In December 2008 the Health Care Assistant Program charged Steven M. Buckminster (HC00131320) with unprofessional conduct. He was found guilty of first degree child rape.
Pierce County
In October 2008 the X-Ray Technician Program denied the application of Benjamin A. Casbarro (XT00006624). He was convicted of conspiracy to import a controlled drug – LSD, disorderly conduct, possession of drug paraphernalia, criminal possession of drug paraphernalia, and criminal possession of dangerous drugs.
In November 2008 the Registered Counselor Program granted the application of Kristina L. Lemon (RC60039997) and placed her registration on probation for three years. She was convicted of possession of controlled substance – methamphetamine. She must comply with terms and conditions set against her registration.
In December 2008 the Nursing Assistant Program charged certified nursing assistant Kori E. Martin (NC10066045) with unprofessional conduct. Martin allegedly failed to respond to department inquiries regarding a complaint.
In December 2008 the Registered Counselor Program granted the application of Dilys Deneise Miller (RC60023969) and placed her registration on probation for two years. She was convicted of unlawful possession of a controlled substance – methamphetamine on more than one occasion. She must comply with terms and conditions set against her registration.
In December 2008 the Nursing Assistant Program charged certified nursing assistant Tiffany M. Quimby (NC10093963) with unprofessional conduct. She allegedly failed to respond to department inquiries regarding a complaint.
Skagit County
In December 2008 the Nursing Commission granted the application of registered nurse Lisa A. Dickinson (RN60049053) and placed her license on probation. Her license to practice as a licensed practical nurse was placed on probation in 2007. She must comply with terms and conditions set against her license.
Snohomish County
In December 2008 the Nursing Commission charged registered nurse Debra S. Nelson (RN00079651) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.
In December 2008 the Health Care Assistant Program charged Andrew D. Pestanio (HC00155217) with unprofessional conduct. He allegedly failed to comply with previous terms and conditions set against his license.
In December 2008 the Registered Counselor Program granted the application of Melody J. Poe (RC60034965) and placed her registration on probation for three years. She was convicted of drunk and disorderly conduct, possession of methamphetamine, criminal mischief, and possession of a controlled substance. She must comply with terms and conditions set against her registration.
In December 2008 the Medical Commission reinstated the license of physician Heidi S. Rendall (MD00019337). Dr. Rendall does not have restrictions against her license.
In December 2008 the Nursing Commission charged registered nurse Deanna Rae Scholl (RN00070969) with unprofessional conduct. She allegedly failed to comply with terms and conditions previously set against her license, and failed to properly respond to calls from patients.
Thurston County
In December 2008 the Board of Pharmacy amended the statement of charges against pharmacist Magdalene Atem-Ossiander (PH00019524). She allegedly changed a prescription to the generic version even though the physician's orders were to fill as written.
In November 2008 the Board of Pharmacy charged pharmacist Donald K. Pohl (PH00065236) with unprofessional conduct. He allegedly failed to comply with previous terms and conditions set against his license. He also allegedly diverted drugs from his place of employment for his own use, and entered into a drug court agreement for theft charges.
Whatcom County
In December 2008 the Office of Emergency Medical Services and Trauma System charged emergency medical technician George M. St. James (ES00131260) with unprofessional conduct. He allegedly pushed his girl friend to the ground, damaged her property, and admitted he was paid to transport marijuana across the Canadian border. He has been charged with assault – domestic violence, malicious mischief – domestic violence and possession with intent to manufacture or deliver a controlled substance.
In December 2008 the Medical Commission entered into an agreed order with physician Roger E. Sharf (MD00029218). He violated boundaries with a patient by entering into a social relationship with the patient and her family. He must comply with terms and conditions set against his license.
Yakima County
In December 2008 the Licensed Mental Health Counselor Program charged Pia P. Hull (LH00007865) with unprofessional conduct. She allegedly referred patients to another licensed mental health counselor and in return for the referral; she received 50 percent of the fees collected from the clients referred.
Out of State
California: In December 2008 the Medical Commission placed the license of physician Khanh Pham (MD00023507) on probation until he has complied with a California order. His license to practice as a physician in California was placed on probation. He must comply with terms and conditions set against his license.
